Doxepin hydrochloride is a tricyclic antidepressant used for the treatment of insomnia characterized by difficulty with sleep maintenance. The recommended dose is 6 mg once daily for adults and 3 mg once daily for the elderly, taken within 30 minutes of bedtime. Doxepin has high binding affinity to the H1 receptor and is extensively metabolized by hepatic enzymes. It is contraindicated in individuals with hypersensitivity to doxepin, untreated narrow angle glaucoma, or severe urinary retention. Common adverse reactions include somnolence, nausea, and upper respiratory tract infection. Special population considerations include use during pregnancy, nursing mothers, pediatric use, and geriatric use.
