Quick Summary

Articaine Hydrochloride and Epinephrine is a local anesthetic used for intraoral submucosal infiltration and nerve block anesthesia in dental procedures. It is indicated for adults and pediatric patients 4 years of age or older. The drug has a rapid onset of action and provides effective anesthesia for up to 1 hour for infiltrations and up to 2 hours for nerve blocks. However, it is contraindicated in patients with hypersensitivity to sulfites and requires caution in patients with severe liver disease, and those taking certain medications such as monoamine oxidase inhibitors and tricyclic antidepressants.

Key Clinical Findings and Indications

  • Local, infiltrative, or conductive anesthesia in dental procedures
  • Indicated for adults and pediatric patients 4 years of age or older
  • Rapid onset of action, with complete anesthesia lasting approximately 1 hour for infiltrations and up to 2 hours for nerve blocks

Important Safety Information

Warning

Contraindicated in patients with hypersensitivity to sulfites, and caution should be exercised in patients with severe liver disease and those taking certain medications

Contraindications

  • Hypersensitivity to sulfites
  • Hypersensitivity to articaine or epinephrine

Adverse Reactions

  • Headache
  • Pain
  • Numbness and tingling
  • Dizziness
  • Nausea and vomiting

Dosing Recommendations

General Guidance

Lower dosages or dosage reduction may be required in debilitated patients, acutely ill patients, elderly patients, and pediatric patients

Infiltration

Adult Dose

0.5 mL-2.5 mL (20 mg-100 mg articaine HCl)

Pediatric Dose

0.5 mL-2.5 mL (20 mg-100 mg articaine HCl), reduced commensurate with age, body weight, and physical condition

Nerve block

Adult Dose

0.5 mL-3.4 mL (20 mg-136 mg articaine HCl)

Pediatric Dose

0.5 mL-3.4 mL (20 mg-136 mg articaine HCl), reduced commensurate with age, body weight, and physical condition

Special Population Considerations

Pregnancy

  • Category C
  • Use during pregnancy only if the potential benefit justifies the potential risk to the fetus

Nursing Mothers

  • Caution should be exercised when administering to nursing mothers
  • Breast milk should be pumped and discarded for approximately 4 hours after injection

Pediatric Use

  • Safety and effectiveness established in pediatric patients 4-16 years old
  • Dosages should be reduced commensurate with age, body weight, and physical condition

Geriatric Use

  • No overall differences in safety or effectiveness were observed between elderly subjects and younger subjects
  • Greater sensitivity of some older individuals cannot be ruled out
