Cefepime

Warning

Hypersensitivity reactions, including anaphylaxis, can occur with cefepime use

Contraindications

• Known hypersensitivity to cefepime or other cephalosporins
• Penicillin or other beta-lactam allergy

Adverse Reactions

• Local reactions, positive Coombs' test, decreased phosphorus, increased ALT and AST, increased PT and PTT, and rash
• Neurotoxicity, including encephalopathy, myoclonus, and seizures, particularly in patients with renal impairment

General Guidance

Dose adjustments recommended for patients with renal impairment, with careful attention to creatinine clearance and potential need for hemodialysis

Pregnancy

• Category B; use during pregnancy only if clearly needed
• No adequate and well-controlled studies in pregnant women

Nursing Mothers

• Cefepime is excreted in human breast milk; caution recommended
• Monitor infant for potential adverse effects

Pediatric Use

• Safety and effectiveness established for patients 2 months to 16 years
• Use in pediatric patients below 2 months not established

Geriatric Use

• Careful attention to dosing and potential adverse effects recommended
• Increased risk of neurotoxicity and other adverse effects in patients with renal impairment
